The Service Provider agrees to provide a comprehensive post-surgical rehabilitation program following total right knee replacement, including initial assessment, fortnightly progress reviews, and up to 20 treatment sessions delivered over a 5-month period.
This agreement shall be effective from December 2, 2025 to May 1, 2026, covering a full rehabilitation cycle of approximately 20 treatment sessions over 5 months.
The Client agrees to pay $640 per month for 5 months ($3,200 total). This includes all treatment sessions, home exercise programming, progress reports, and one discharge ergonomic consultation.
Client may cancel with 14 days written notice. Unused sessions will be refunded at the per-session rate. Appointments cancelled without 24-hour notice will be charged at 50% of the session rate.
All clinical records and assessment data remain the property of Chen Physiotherapy & Rehab. The treatment program will be adapted in response to clinical progress and any changes in medical status. Client agrees to complete home exercise programs as prescribed between sessions.
