The Service Provider (Monroe Nutrition) agrees to provide nutrition coaching services to the Client, including bi-weekly coaching sessions, meal planning guidance, habit tracking support, and email check-ins between sessions.
The services provided are nutrition coaching and general wellness education. The Service Provider is a Certified Nutrition Coach, not a licensed dietitian, physician, or medical professional. These services do not constitute medical nutrition therapy, diagnosis, or treatment. The Client should consult a licensed healthcare provider for any medical concerns.
This agreement is effective from January 1, 2026 to June 30, 2026, for a total of 6 months of ongoing coaching support.
The Client agrees to pay $297 per month ($1,782 total) for the duration of this agreement. Payment is due on the 1st of each month.
Sessions may be rescheduled with 24 hours notice. Sessions cancelled with less than 24 hours notice are forfeited. Either party may terminate this agreement with 14 days written notice.
